Provider Manual

A provider manual is an extension of the physician or provider agreement and furnishes the provider and staff with information about ChoiceCare Network policies and procedures, claims, and guidelines.

To comply with the Policies and Procedures clause of the physician or provider contract, please obtain a provider manual for each Payor to this network to determine which policies and procedures are applicable to each Payor.

To comply with the Policies and Procedures clause where Humana is the Payor, the physician or provider must also abide by all provisions contained in all applicable sections of Humana's Provider Manual.
